Q: Birth registration application process?
Ans: You have to apply in the prescribed application form for birth registration (whether printed or hand written) to the Registrar along with the following documents or certificates. You can also apply for birth registration online along with the concerned registrar's office by entering the br.lgd.gov.bd website. The registrar will be able to register the birth by submitting the print copy of the application to the registration office ( click to apply online ).

If the application is made within five years of the birth of the registered person-
- Data collector certification, or
- Attested copy of EPI card, or
- Certificate of clearance from the concerned medical institution or attested copy of the birth certificate obtained from that institution, or
- Attested copy of any other birth certificate as the registrar deems necessary, or
- Certification of any NGO worker specified by the Registrar as a data collector.
- Certification of MBBS doctor for proof of age and proof of place of birth or place of permanent residence for the concerned ward councilor / member, or
- Certification of the principal or head teacher of the concerned educational institution or the teacher or officer nominated by him / her as the data collector for proof of age and place of birth, or
- EPI card or passport or secondary school certificate or attested copy of birth certificate of any medical institution or birth certificate obtained from that medical institution for proof of age and place of birth, or
- Attested copy of any other birth certificate as the registrar deems necessary, or
- Certification of any NGO worker specified by the Registrar as a data collector.
Ans: Birth can be registered with the registrar of the place of birth or permanent address of the person or any place where he / she is currently residing and death should be registered with the registrar of the area where the person will die. By:
- The chairman of the union council or any official or member authorized by the government;
- The mayor of the municipality or any other officer or councilor empowered by him;
- The mayor of the city corporation or any officer or councilor authorized by him;
- The President of the Cantonment Board or any officer authorized by him; Oh
- Ambassador of Bangladesh Embassy abroad or any official authorized by him.
- Zonal Executive Officers in Dhaka South City Corporation, Dhaka North City Corporation and Gazipur City Corporation areas are Councilors of General Wards in Chittagong, Khulna and Rajshahi City Corporations, Officers of Barisal, Sylhet and Rangpur City Corporations in Municipal Offices and Municipal Corporations Performing duties.
